Instructions: Appointments must be scheduled online. The more detailed your description of the health issue, the easier it will be for the hospital to evaluate your case. The hospital will contact you once your consultation is scheduled.

FIRST CONSULTATION - Upon arrival at a SARAH Network unit, go to the Reception Desk. The receptionist will open your medical record — a folder that will contain information about your health condition. The medical team will record all data related to your treatment in this folder. For admission to the SARAH Network, adults must present an ID document and CPF (Brazilian taxpayer number); for children, a birth certificate is required. You will then receive a SARAH Hospital card, which will serve as your identification for all appointments and will also display the dates of your future consultations. If you are unable to attend, please notify the hospital and request a rescheduling. If possible, bring all previously performed medical exams to your first appointment.
