1. Neurology Consultation for Diagnosis of Unknown Conditions or Treatment of Disorders such as Ataxias, Dystrophies, Multiple Sclerosis, etc.
Other patients may also be seen.
The patient must bring or send by mail two copies of the following documents:

For children without an RG, a birth certificate is required (mandatory for children under 3)
A referral to UNEPE signed by a healthcare professional (physiotherapist, physician, etc.) is also required.
An appointment will be scheduled and a card with the appointment date will be issued. Please confirm all contact phone numbers.
Note: Dr. César Nicolatti, neurologist, also provides botulinum toxin treatment for residents of the state.
2. Physiatry Consultation for Evaluation of Botulinum Toxin Treatment
To speed up appointments for patients needing treatment for spasticity and/or dystonia (e.g., stroke, TBI, spinal injury, cerebral palsy, adrenoleukodystrophy, blepharospasm, spasmodic torticollis, neurological injury from gunshot wounds, including ataxia patients), there is a separate waiting list for evaluations with physiatrists Dr. Afonso or Dr. Regina.
The procedures to schedule this consultation are the same as for regular consultations, as explained in item 1.
A referral to UNEPE must be signed by any healthcare professional (e.g., physiotherapist or physician), requesting treatment for spasticity or dystonia.
As the botulinum toxin is provided by the State Health Department, it cannot be given to patients residing in other states.
Patients will be added to the new list and notified of the expected date or month when the physiatrists will be in Campo Grande. Please be sure to provide all possible phone numbers. Once the physiatrists confirm the evaluation date, UNEPE will contact the patient by phone to inform them of the exact date and time of their appointment.
Due to high demand, we recommend opening a patient file as early as possible to ensure faster service. After the evaluation, if botulinum toxin application is needed, UNEPE will submit the request to Casa de Saúde, and the application will be scheduled within 1 to 2 months. Following the application, the patient must wait the period indicated by the physician and call UNEPE to schedule a follow-up evaluation.
Note: All patients scheduled for botulinum toxin application or evaluation on a given day will be seen during the time the physiatrists are in Campo Grande (usually two days). As appointments are on a first-come, first-served basis, delays may occur. We provide water, a TV, and wheelchairs. We recommend bringing snacks and any necessary medications, especially for patients in more fragile conditions.
